Activity-based locomotor training
Activity-based locomotor training
This map follows this approach and explains it below. It holds no study of its own about it yet, so there are no measurements, no evidence states and no readiness on this page — an explanation is not evidence, and this page will not print one as the other.
What this is
Repeating a movement many times — stepping on a treadmill with part of the body weight supported, or practising specific tasks — performed as close to the natural movement as possible.
What problem it is trying to solve
After an injury the nervous system and the muscles quickly lose what is not used. This approach tries to use the capacity of surviving circuits to reorganise in response to repetition.
How it is supposed to work
The proposed mechanism is that repeated signals from the limb and from weight-bearing change how the networks in the cord below the injury behave, even without a command from above.
Limits and known risks
It is neither surgery nor a drug, so the risks are different: overloading, falls, fatigue, pain. It needs many hours, equipment and staff, and it is not available everywhere. Training is also the ordinary comparator other interventions are measured against, which makes its effect and a device's effect easy to confuse.
What is still missing
What is missing: an agreed dose — how many hours, how often, for how long — and measurements that can be compared between centres.

Nothing has been retrieved and curated for it. That is a fact about this corpus, not about the science: work on this approach exists and is simply not held here yet.
